Autopsy: Edinburgh girl died from surgery

The autopsy of a 17-year-old Edin­burgh girl showed she died from hemorrhaging following surgery-related complications.

Deputy Coroner Larry Fisher received the autopsy report on Erin Streeval on Tuesday from Indiana University Medical Center in Indian­apolis.

Streeval, a senior at Southwestern High School in Shelby County, died Oct. 13 at Columbus Regional Hospital, where she had been admitted Oct. 9 for gallbladder surgery.

Dr. David Thompson of Southern In­diana Surgery performed the surgery. The physicians at Southern Indiana Sur­gery released the following state­ment: “Dr. David Thompson and all of the surgeons of Southern Indiana Surgery are deeply saddened by the death of Erin Streeval. It is impossible to suf­ficiently express the grief we feel and the pain we know her family and fiancé are experiencing. For nearly a decade, the surgeons of Southern Indiana Sur­gery have successfully performed over 3,000 laparoscopic surgeries. Yet the loss of this one precious life is not consolable. Because of the laws gov­erning patient confidentiality, Southern Indiana Surgery is unable to provide details concerning any specific patient. The surgery group is, however, fully participating in the peer review process that is currently under way.”

Thompson still has hospital privileges pending the investigation, said Doug Leonard, chief executive officer of CRH. If the incident had been of such a nature to call for a summary suspens­ion, that would have occurred, he said. “This is a tragic event, and we in no way want to minimize that,” Leonard said. “The potential for complications exist with any surgery. No surgery is routine, and death is a very rare and tragic result of some surgical proce­dures.” Every surgical case, whether a death has occurred or not, is reviewed because every case has the potential for complications, Leonard said. When a death occurs, a root cause analysis and peer review investigation have to be conducted within 45 days. For the peer review portion of the investigation, information on the incident is sent to a surgeon outside the hospital. “We’re working to send that record out now,” Leonard said.

Also, a Quality Management Com­mittee, composed of an interdisciplinary team of hospital medical staff, analyzes the incident. The Quality Management Committee looks at the individual case for problems, but it also looks for patterns of problems, Leonard said. Ev­ery surgical case, whether a death has occurred or not, is reviewed because every case has the potential for com­plications, Leonard said. When a death occurs, a root cause analysis and peer review investigation have to be con­ducted within 45 days.

She was born May 25, 1983, in Franklin. Her parents are Carl Streeval of Martinsville and Cindy (Hancock) Streeval of Edinburgh. Other survivors include maternal grandparents, Mr. and Mrs. James A. Hancock of Edinburgh; paternal grandparents, Ulysses and Bet­ty Streeval of Edinburgh; great-grand­mother, Mary Streeval of Edinburgh; a half-brother, Conner Streeval of Colum­bus; a half-sister, Teaneal Denny of Edinburgh; a fiancé, Lucas Simpson of Edinburgh; two uncles; and one aunt.

She was a senior at Southwestern High School in Shelbyville.

She was planning to be married in February and to attend college.

The Rev. Lewis Burton will conduct a service at 11 a.m. Tuesday at Who-So-Ever-Will Church in Edinburgh. Friends may call from 4 to 8 p.m. today at the church. Burial will be at Rest Haven Cemetery in Edinburgh. Arrangements are being handled by Eskew-Eaton Funeral Home in Edin­burgh.
