THE COMMONWEALTH OF KENTUCKY
WHEREAS, Hezekiah Maddox, has been appointed by the County Court of Campbell County, and has qualified as Guardian to Sarah M Smith, a minor.
NOW, we Hezekiah Maddox as principal, and Alexander Caldwell, his sureties, do hereby covenant to and with the Commonwealth of Kentucky, that the said Hezekiah Maddox, will faithfully discharge the trust of Guardian to said minor, in all respects as required by law.
